ADENOSINE (Adenocard)

Theoretically, Ilex guayusa might decrease the vasodilatory effects of adenosine and interfere with its use prior to stress testing.
Ilex guayusa contains caffeine. Caffeine is a competitive inhibitor of adenosine at the cellular level. However, caffeine does not seem to affect supplemental adenosine because high interstitial levels of adenosine overcome the antagonistic effects of caffeine. It is recommended that methylxanthines such as caffeine, as well as methylxanthine-containing products, be stopped 24 hours prior to pharmacological stress tests. However, methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.

Here's the thing about adenosine: it's almost always given as an injection in a hospital or clinic, often during a heart stress test or to quickly fix a fast heart rhythm. The caffeine in guayusa works in your body as a competitor that can blunt how adenosine acts on your heart and blood vessels, so in theory drinking it could weaken adenosine's effect.

In real life, the doses of adenosine pushed during a procedure are high enough to mostly overpower caffeine, so this is more of a precaution than a proven problem. Still, clinics usually ask you to skip caffeine (including guayusa) for about 24 hours before a stress test so your results are accurate. Please follow your care team's instructions and tell them what you drink.
