Coffee has a moderate interaction with Adenosine. Theoretically, coffee might decrease the vasodilatory effects of adenosine and interfere with its use prior to stress testing.

ADENOSINE (Adenocard)
Coffee contains caffeine. Caffeine is a competitive inhibitor of adenosine at the cellular level. However, caffeine does not seem to affect supplemental adenosine because high interstitial levels of adenosine overcome the antagonistic effects of caffeine. It is recommended that methylxanthines such as caffeine, as well as methylxanthine-containing products, be stopped 24 hours prior to pharmacological stress tests. However, methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing to know: adenosine is usually given by IV during a heart stress test or to quickly correct a fast heart rhythm. The caffeine in your morning coffee actually competes with adenosine at the same cellular receptors, so in theory it could blunt how well adenosine widens your blood vessels and stresses the heart for the test. That could make the test less accurate.
In practice, the doses of adenosine used in the hospital are high enough to usually override caffeine's blocking effect, so this is more of a precaution than a strong, proven problem. Still, most centers ask you to skip coffee, tea, and other caffeine for about 24 hours before a stress test. Always follow the prep instructions your doctor or pharmacist gives you.
