Why this applies to Apixaban
Apixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Ilex Guayusa interacts with.
Ilex guayusa contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, caffeine in Ilex guayusa might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Here's the gist: Eliquis (apixaban) already thins your blood to stop dangerous clots, so anything that nudges bleeding risk up a notch is worth knowing about. Guayusa's main active ingredient is caffeine, and caffeine has some mild antiplatelet activity, meaning it can make platelets a little less sticky. Stack that on top of apixaban and, in theory, you could be slightly more prone to bleeding.
I want to be honest about how thin this evidence is. This concern comes from lab pharmacology, not real human reports, so it's mostly theoretical. A cup or two of guayusa tea is unlikely to cause trouble. Still, watch for easy bruising, nosebleeds, or bleeding gums, and check with your pharmacist or doctor before using concentrated supplement doses.

Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. What Is Apixaban Used For?
Brand name: Eliquis
Apixaban tablets (sold as Eliquis and under the generic name apixaban) are used to reduce the risk of stroke and systemic embolism (clots traveling to other parts of the body) in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs), as well as to lower the risk of those clots coming back.
Eliquis is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, following at least 5 days of initial anticoagulant treatment.
