Theoretically, Ilex guayusa may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Ilex guayusa contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, caffeine in Ilex guayusa might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Here's the situation with guayusa and urokinase. Urokinase is a powerful clot-busting drug given in a hospital setting to dissolve dangerous blood clots, so on its own it already raises your bleeding risk. The caffeine in guayusa has been reported to have mild blood-thinning (antiplatelet) effects, and on paper that could nudge your bleeding risk a little higher.

That said, this is a theoretical concern based on how caffeine behaves, not something that has actually been seen in real patients. Realistically, urokinase is something you'd receive under close medical supervision, so the team is already watching you carefully for bruising, bleeding, or oozing. Casual amounts of a guayusa tea are unlikely to be a big deal, but it's always worth mentioning any supplements to your care team.
