Why this applies to Valproic Acid

Valproic Acid falls under CNS DEPRESSANTS — the group Kratom interacts with.

Theoretically, concomitant use might increase the risk for adverse CNS depressant effects.
Kratom contains mitragynine, a mu-receptor agonist that may cause respiratory depression. Although results from animal research suggest that mitragynine causes less respiratory depression than the CNS depressant codeine, fatalities have been reported for patients who ingested a combination of kratom and O-desmethyltramadol, another mu-receptor agonist. Additionally, observational research suggests that adverse effects such as drowsiness and coma are more likely to occur in individuals taking kratom in combination with agents such as opioids and sedatives. Theoretically, ingesting kratom along with other CNS depressants can increase the risk of adverse effects, including drowsiness, coma, and/or severe or fatal respiratory depression.

Here's the gist with valproic acid (Depakene) and kratom. Valproic acid calms down electrical activity in the brain to control seizures and stabilize mood. Kratom acts on the brain's opioid receptors and can have a sedating, slowing effect of its own. Put the two together and you may simply pile on more central nervous system slowdown, meaning extra drowsiness, dizziness, foggy thinking, and slowed breathing.

I'll be honest, this concern is mostly theoretical, based on how both work rather than solid studies in people on valproic acid. But kratom itself is not regulated or proven safe, and serious harm has been reported when it's mixed with other sedating substances. Please loop in your doctor or pharmacist before combining them.

Theoretically, taking kratom in combination with serotonergic drugs may increase levels of serotonin, increasing the risk of serotonin syndrome.
A 63-year-old male presented with possible serotonin syndrome after taking an unknown dose of kratom for up to 3 months along with serotonergic prescription medications bupropion, buspirone, desvenlafaxine, trazodone, and ziprasidone. It was hypothesized that kratom inhibited the cytochrome P450 3A4-, 2C9-, 2D6-, and 1A2-mediated metabolism of several of these serotonergic medications, consequently increasing systemic exposure to serotonin.

Here's the honest picture: the interaction we have on file is grouped under drugs that raise serotonin, and valproic acid is not really a serotonin drug. It's an anticonvulsant that works mainly by boosting GABA in the brain, so the classic serotonin syndrome concern doesn't fit it well. This particular warning is also theoretical and based on a single case report, not solid studies.

That said, mixing kratom with valproic acid still isn't something I'd take lightly. Both can cause drowsiness, dizziness, and slowed breathing, and that can add up. Kratom carries real risks of dependence and harm on its own. Please loop in your pharmacist or doctor before combining the two so they can look at your full medication list.

Valproic acid capsules (brands Valproic and Valproic Acid) and valproic acid oral solution (brand Valproic Acid) are both used to treat epilepsy. Specifically, they are approved for complex partial seizures (either alone or with other seizure types) as monotherapy or add-on therapy, and for simple and complex absence seizures as sole therapy or add-on therapy.

Both forms are intended for adults and pediatric patients. Always follow your prescriber's guidance on which product and regimen is right for you.
