Ryzodeg 70/30 combines two insulins (aspart and degludec) that already do the heavy lifting of lowering your blood sugar. Here's the concern: in animal studies, Madagascar periwinkle also lowered blood glucose. So if you were to take this herb alongside your insulin, the two effects could stack up and push your sugar too low.

I want to be honest about how strong this evidence is, it comes only from diabetic rats, not people, so it's theoretical at this point. Still, with insulin you don't want surprises. Watch for shakiness, sweating, dizziness, confusion, or a racing heart, which can signal low blood sugar. Honestly, the raw plant itself is toxic and isn't safe to take, so please check with your pharmacist or doctor before combining anything with your insulin.
