Sitagliptin falls under ANTIDIABETES DRUGS — the group Maitake Mushroom interacts with.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind: maitake mushroom may have its own blood sugar lowering effect. Some studies in people with type 2 diabetes found that maitake polysaccharide nudged glucose levels down. Sitagliptin (Januvia) already works to lower your blood sugar, so stacking the two together could push your numbers lower than expected.
This isn't a breakdown or enzyme issue. It's more that two blood-sugar-lowering things may add up. Sitagliptin alone rarely causes low blood sugar, but with maitake you may want to watch for shakiness, sweating, dizziness, or feeling foggy. The evidence here is fairly limited, and food amounts of the mushroom are unlikely to be a problem. Before starting a concentrated supplement, please check with your pharmacist or doctor so they can help you monitor.

Sitagliptin is used alongside diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus. It is available as tablets (Januvia, Zituvio, and generic sitagliptin products) and as an oral solution (Brynovin), all approved for this same use.
Sitagliptin is not for type 1 diabetes. It has not been studied in people with a history of pancreatitis, so its safety in that group is unknown.
