Dyphylline, Ephedrine, Guaifenesin, Phenobarbital falls under STIMULANT DRUGS — the group Methylsynephrine interacts with.

Here's the thing to focus on with Lufyllin-EPG: it already contains two stimulant ingredients. Ephedrine acts like adrenaline on your heart and blood vessels, and dyphylline (a theophylline-type drug) also speeds the heart and stimulates the system. Adding methylsynephrine, another adrenaline-like stimulant, simply stacks more of the same effect on top. The concern, which is based on how these drugs work rather than hard studies, is a higher chance of a racing or pounding heartbeat, jitteriness, trouble sleeping, and most importantly raised blood pressure.
The guaifenesin and phenobarbital parts aren't the worry here; it's the stimulant pile-up. Since methylsynephrine isn't even a legal supplement ingredient in the U.S., I'd steer clear of it. Please check with your pharmacist or doctor before combining anything like this.
