In vitro research shows that neem leaf extract inhibits CYP1A2 enzymes. So far, this reaction has not been reported in humans.

Here's the piece of this combo product that matters most with neem: the caffeine. Caffeine is cleared mainly by a liver enzyme called CYP1A2. In test-tube studies, neem leaf extract slows that enzyme down, which could let caffeine linger longer in your body. In plain terms, you might feel more of caffeine's usual effects, like jitteriness, a racing heart, trouble sleeping, or an upset stomach.
I want to keep this in perspective, though. This is only lab-based evidence and hasn't actually been seen in people, so it's more theoretical than proven. The aspirin and dihydrocodeine pieces aren't really driven by this enzyme. If you take concentrated neem supplements regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

Here's the thing about this combo pill (Synalgos DC): the dihydrocodeine part is the piece that interacts with neem. In a test tube, neem leaf extract has been shown to slow down a liver enzyme called CYP3A4, which helps clear dihydrocodeine. If that enzyme runs slower, more of the opioid could hang around, which in theory could nudge up its sedating side effects, things like drowsiness, dizziness, constipation, or slowed breathing.
I want to be honest with you though: this is based only on lab research, not on real people. It has never actually been reported in humans, so I wouldn't lose sleep over it. The aspirin and caffeine parts aren't really the concern here. Still, since this medicine already carries opioid risks, just give your pharmacist or doctor a heads-up before adding neem.
