Demeclocycline falls under P-GLYCOPROTEIN SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits renal P-glycoprotein transport activity. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with neem and your demeclocycline. Demeclocycline is a tetracycline antibiotic that your kidneys help clear using a transporter called P-glycoprotein. In lab dishes, neem leaf extract seems to block that transporter. If that same effect happened in people, your body might hold onto a little more of the drug, which could mean stronger effects and more side effects like stomach upset, nausea, sun sensitivity, or dizziness.
I want to be honest about the strength of this, though. This is only test-tube and animal data, and it has never actually been reported in real patients. So I wouldn't lose sleep over it, but since neem can also irritate the gut, it's worth a quick word with your pharmacist or doctor before combining them.

What Is Demeclocycline Used For?
Brand name: Declomycin
Demeclocycline Hydrochloride tablets are used to treat bacterial infections caused by susceptible organisms. These include Rocky Mountain spotted fever, typhus, Q fever, tick fevers, respiratory tract infections (including those caused by Mycoplasma pneumoniae), cholera, plague, tularemia, brucellosis, relapsing fever, chancroid, and sexually transmitted infections such as lymphogranuloma venereum, nongonococcal urethritis, and gonorrhea (when penicillin cannot be used).
It is also used for certain skin and skin structure infections, bartonellosis, granuloma inguinale, actinomycosis, listeriosis, anthrax (when penicillin is contraindicated), and as an adjunct in acute intestinal amebiasis. In severe acne, it may be used as part of combination therapy. It should only be used for infections that are proven or strongly suspected to be caused by bacteria that respond to it.
