Duvelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with Duvelisib (Copiktra): your body relies on a liver enzyme called CYP3A4 to break it down and clear it out. Lab studies suggest that neem leaf extract can slow that same enzyme. If neem does that in real life, Duvelisib could build up to higher levels than expected, which may mean stronger effects and more side effects (think diarrhea, mouth sores, low blood counts, infections, or liver irritation).

I want to be honest about the evidence here: this is only seen in test-tube research, not in people, so it's theoretical. Still, Duvelisib is a serious cancer drug, so don't add neem on your own. Please check with your oncology team or pharmacist first.

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
