Why this applies to Naloxegol
Naloxegol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Movantik (naloxegol). Your body clears this drug using a liver enzyme called CYP3A4. In lab dish studies, neem leaf extract seemed to slow that same enzyme down. If that happened in real life, naloxegol could build up a bit more than expected, which might mean stronger or more frequent side effects like belly cramping, diarrhea, nausea, or stomach pain.
The important part: this is only theoretical, seen in test tubes, never actually reported in people. So I wouldn't panic. Still, since naloxegol already warns against combining it with strong CYP3A4 blockers, it's worth a quick chat with your pharmacist or doctor before adding concentrated neem supplements, just to be safe.

What Is Naloxegol Used For?
Brand name: Movantik
Movantik (naloxegol) is used to treat opioid-induced constipation (OIC) — constipation caused by opioid pain medications — in adults who have chronic non-cancer pain. It is also approved for adults with chronic pain related to prior cancer or its treatment, as long as they don't need frequent (roughly weekly) increases in their opioid dose.
It is not indicated for constipation from other causes, and it is not approved for use in children.
