Here's the thinking on combining neem with Enspryng (satralizumab). Satralizumab calms down an overactive immune system to protect your eye nerves and spinal cord in NMOSD. Neem, on the other hand, has shown in animal and lab studies that it might do the opposite and stimulate the immune system. So the concern is that neem could work against your medication and make it less effective, possibly leading to a higher chance of an NMOSD flare.
I'll be honest with you though: this is theoretical and based only on animal and test-tube data, not on people taking these two together. So it's a 'be cautious' situation, not a panic one. Satralizumab isn't a prodrug, so this isn't about how it's broken down. Before adding neem in any concentrated supplement form, please run it by your neurologist or pharmacist first.

Satralizumab-mwge injection is used to treat neuromyelitis optica spectrum disorder (NMOSD; an autoimmune disorder of the nervous system that affects eye nerves and the spinal cord) in certain adults. Satralizumab-mwge is in a class of medications called interleukin-6 (IL-6) receptor inhibitors. It works by blocking the activity of the part of the immune system that may damage certain areas of the nervous system in people with NMOSD.
