Sildenafil Citrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Viagra (sildenafil): your body relies on an enzyme called CYP3A4 to break it down and clear it out. Lab studies suggest neem leaf extract may slow that enzyme down. If that happened in real life, sildenafil could hang around longer and build up, which means stronger effects and a bigger chance of side effects like headache, facial flushing, a stuffy nose, dizziness, or low blood pressure.
The important thing to know is that this is theoretical. It comes from test-tube research, not from real people, so we honestly don't know if it matters when you actually take both. I wouldn't lose sleep over it, but since neem can also be risky to swallow, please run it by your pharmacist or doctor first.

Here's the gist for Viagra (sildenafil). Your body relies partly on a liver enzyme called CYP2C9 to clear this medication. In lab dish studies, neem leaf extract appears to slow that enzyme down. If that held true in people, sildenafil could hang around longer and hit a bit harder than expected.
What might that look like? The usual sildenafil effects, just turned up: headache, flushing, a stuffy nose, dizziness, or a drop in blood pressure. The important part is this is purely theoretical. It comes from test-tube work and has never actually been reported in a real person, so I wouldn't lose sleep over it. Still, if you take concentrated neem supplements, it's worth a quick word with your pharmacist or doctor before combining them.

Sildenafil treats two distinct conditions depending on the product. Viagra (tablet) and Vybrique (dissolving oral film) are approved for erectile dysfunction (ED) — difficulty getting or keeping an erection. Generic sildenafil tablets and oral film are also approved for ED.
Revatio and several generic sildenafil products (tablets, oral suspension, and injection) are used for pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs — in adults to improve exercise ability and slow the disease from getting worse. Revatio oral suspension and injection are also approved for PAH in children ages 1 to 17.
