Tramadol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP3A4 enzymes. So far, this reaction has not been reported in humans.

Here's the gist with neem and tramadol. Lab studies (in test tubes, not people) suggest neem leaf extract may slow down a liver enzyme called CYP3A4. Tramadol is partly handled by that same enzyme, so in theory neem could let a bit more tramadol linger in your system. That might nudge up its effects and side effects, like extra drowsiness, dizziness, nausea, constipation, or feeling unusually foggy.

I want to be honest though: this is only a theoretical concern that hasn't been seen in actual patients, so I wouldn't panic. Still, since tramadol affects the brain and breathing, it's worth a quick chat with your pharmacist or doctor before pairing them, especially with concentrated neem supplements rather than a dab of neem skin product.
