Why this applies to Tretinoin

Tretinoin falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.

Here's the gist for tretinoin, the oral capsule version (Vesanoid) used for acute promyelocytic leukemia. Your body partly clears tretinoin using a liver enzyme called CYP2C8. In a lab dish, neem leaf extract slowed that same enzyme down. If that happened in a real person, tretinoin could clear a bit more slowly and build up, which might mean stronger effects and more side effects like headache, dry skin and lips, bone pain, or higher triglycerides.

I want to be honest, though: this is theory only, based on test-tube work, and it has never actually been reported in people. Still, because tretinoin is a serious chemotherapy medicine managed closely by your cancer team, please don't add neem without first checking with your oncologist or pharmacist.

Tretinoin cream 0.02% is approved to help lessen the look of fine facial wrinkles in people who also follow a comprehensive skin care and sun-avoidance program — including daily sunscreen, protective clothing, and moisturizers.

It's important to know what tretinoin does not do: it will not erase wrinkles, reverse sun damage, repair deeply weathered skin, or restore younger-looking skin. It has also not been shown to help with coarse or deep wrinkles, rough skin texture, dark spots, or sagging skin.
