Theoretically, noni may increase the risk of hypotension when used in combination with antihypertensive drugs.

Here's the simple version: cilazapril is a blood pressure medicine (an ACE inhibitor), and noni juice may have a mild blood-pressure-lowering effect of its own. Put them together and those effects can add up, so your numbers could drop a little lower than expected.

If that happens, you might notice dizziness, lightheadedness, feeling faint when you stand up, or tiredness. I want to be honest about the evidence though: this is mostly theoretical and based on weak, anecdotal reports, so for most people it's a gentle nudge, not a danger. Still, if you'd like to try noni, mention it to your pharmacist or doctor first, and keep an eye on your blood pressure when you start. Don't change your cilazapril on your own.

Theoretically, combining noni and ACE inhibitors might increase the risk of hyperkalemia.
Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with ACE inhibitors, which can also increase potassium levels.

Here's the thing to keep in mind if you take cilazapril (Inhibace) for your blood pressure. Like other ACE inhibitors, cilazapril tends to nudge your potassium levels up a bit on its own. Noni juice happens to be fairly rich in potassium (roughly 6 mEq per 100 mL), so drinking a lot of it alongside cilazapril could push your potassium higher than you'd want.

I'll be honest, this concern is mostly theoretical and based on weak, anecdotal evidence, not strong studies. The bigger risk is with concentrated supplement doses or large daily amounts of juice rather than an occasional glass. High potassium (hyperkalemia) can cause muscle weakness, tingling, or an irregular heartbeat. If you have any kidney trouble or want to use noni regularly, please check with your pharmacist or doctor first so they can keep an eye on your potassium.
