Here's the simple version: your birth control pill (drospirenone with ethinyl estradiol) is processed through your liver, and on rare occasions noni juice or noni supplements have been linked to liver irritation. The worry is that putting two things that can stress the liver together might add up. This isn't a strong, well-proven concern, it's based mostly on scattered case reports, so I don't want you to panic.

Still, it's worth being a little cautious. If you ever notice yellowing of your skin or eyes, dark urine, unusual tiredness, nausea, or pain in the upper right side of your belly, check in with your doctor. Before adding regular noni supplements, just run it by your pharmacist or prescriber first.

Noni juice contains significant amounts of potassium, about 6 mEq/100 mL juice. This may increase the risk for hyperkalemia when used in conjunction with potassium-sparing diuretics, which can also increase potassium levels.

Here's the connection most people don't expect. The drospirenone in your birth control pill (brands like Yasmin or Gianvi) acts a bit like a mild potassium-sparing water pill, so it can nudge your potassium level up slightly. Noni juice happens to be fairly rich in potassium too, around 6 mEq per 100 mL. Put the two together and, in theory, your potassium could climb higher than it should, which doctors call hyperkalemia.

I'll be honest, this is mostly a theoretical concern based on anecdotal reports, not strong studies, and most healthy people handle it fine. But if you have kidney trouble or take other medicines that raise potassium, it matters more. A quick chat with your pharmacist or doctor (and maybe a simple blood test) clears it up.
