Opsynvi combines two medicines that both lower blood pressure in the lungs (and the body overall). Macitentan relaxes blood vessels, and tadalafil (the same drug found in Cialis) does too. So they already nudge your blood pressure downward on purpose. Some early research hints that noni juice may lower blood pressure on its own, so stacking it on top could push your pressure a little too low.

If that happens, you might feel dizzy, lightheaded, faint, or unusually tired, especially when you stand up. To be fair, this concern is based only on weak, anecdotal evidence, not strong studies. Still, since you're on a drug made for a serious lung and heart condition, it's worth a quick chat with your pharmacist or doctor before adding noni.

Here's the thing to know about Opsynvi, which combines macitentan and tadalafil: the macitentan piece already carries a known caution about affecting the liver, and your doctor may check liver-related bloodwork while you're on it. Noni juice has been tied, in rare case reports, to liver trouble of its own. So the worry isn't that noni changes how the drug works. It's that stacking two things that can each stress the liver might add up.

I'll be honest: this is based on scattered anecdotal reports, not strong studies, so it's more of a caution than a red alert. Still, watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and don't add noni without checking with your pharmacist or doctor first.
