Here's the gist with this cough and congestion combo. The dextromethorphan part of Vicks Formula 44D is cleared from your body by a liver enzyme called CYP2D6. Phellodendron contains berberine, which lab studies and some early human data suggest can slow that same enzyme down. So in theory, taking them together could let dextromethorphan build up a bit more than usual, possibly leading to extra drowsiness, dizziness, or stomach upset.

I want to be honest that this is a theoretical concern, based mostly on lab work, not strong real-world reports, so it's not something to panic over. Still, if you're already taking phellodendron regularly, it's worth a quick chat with your pharmacist or doctor before adding this cough syrup, rather than changing anything on your own.

Phellodendron contains berberine, which can mildly slow that enzyme down. If the enzyme works slower, dextromethorphan can hang around longer than usual, which could mean more drowsiness, dizziness, or feeling foggy. The phenylephrine (the decongestant) isn't really affected by this same pathway.

I want to be honest: this is a theoretical concern based on lab work, not strong human studies, so for most people taking the cough syrup as directed it's unlikely to cause real trouble. Still, since you're combining them, it's worth a quick chat with your pharmacist before you start.

Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit cytochrome P450 2D6 (CYP2D6) activity and reduce the metabolism of dextromethorphan. Theoretically, phellodendron may also inhibit the metabolism of dextromethorphan.
