Lonafarnib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Phellodendron interacts with.
Phellodendron contains berberine. Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, phellodendron might also inhibit CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Lonafarnib (Zokinvy), the medicine used for progeria. One of the enzymes that helps your body break it down is called CYP2C9. The berberine in Phellodendron may slow that enzyme down a bit, at least based on early lab and pharmacology data. If it does, Lonafarnib could build up in your blood and hit harder than intended.
For Lonafarnib, that could mean more nausea, vomiting, diarrhea, poor appetite, or fatigue, and it can also affect heart rhythm (QT prolongation). This is a theoretical concern, not something proven in people, so don't panic. Still, given how carefully Lonafarnib is dosed, please run any Phellodendron product past your pharmacist or doctor first.

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Zokinvy (lonafarnib). Your body relies heavily on a liver enzyme called CYP3A4 to break this medicine down and keep its level in check. The berberine inside phellodendron may slow that enzyme down a bit. If clearance slows, more lonafarnib can hang around in your bloodstream, which could nudge up its typical side effects like nausea, vomiting, diarrhea, poor appetite, fatigue, or changes in lab values your doctor watches.
I want to be honest about the evidence: this is theoretical, based on how the chemistry should behave, not on real cases in people taking these two together. Still, lonafarnib is a serious, carefully dosed drug, so please don't add phellodendron without first checking with your prescribing doctor or pharmacist.

What Is Lonafarnib Used For?
Brand name: Zokinvy
Lonafarnib (sold as Zokinvy capsules) is used in people 12 months of age and older with a body surface area of at least 0.39 m². Body surface area is a measure of body size based on height and weight.
It is used to reduce the risk of death in Hutchinson-Gilford Progeria Syndrome (HGPS). It is also used to treat processing-deficient progeroid laminopathies with a specific gene change. It is not for other progeroid syndromes or processing-proficient progeroid laminopathies.
