Lonafarnib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Yohimbe interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.
In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes. Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

Here's the gist with this pairing. Your body clears yohimbine (the active part of yohimbe) using a liver enzyme called CYP3A4. Lonafarnib (Zokinvy) blocks that same enzyme, so it can act like a traffic jam, letting yohimbine build up higher than it normally would. More yohimbine in your system means a bigger chance of its rough side effects, things like a racing heart, jittery or anxious feelings, and spikes in blood pressure.

I'll be honest, this is a theoretical concern based on how the drugs work, not on real-world reports, so I don't want to overalarm you. Still, since over-the-counter yohimbe doses are unpredictable and these side effects can be serious, please check with your pharmacist or doctor before combining them.

Here's the gist for Zokinvy (lonafarnib), the medication used to help children with progeria live longer. Your body relies on an enzyme called CYP3A4 to handle lonafarnib. Some early lab research suggests yohimbe may speed that enzyme up, which in theory could clear lonafarnib out faster and leave less of it working in the body. Lonafarnib is the active drug itself, not a prodrug, so faster clearance would mean a weaker effect, not a stronger one.

I want to be honest about how thin this evidence is. It comes only from test-tube studies, not from people, so the real-world impact is uncertain. Still, because lonafarnib is a serious medication, please loop in your doctor or pharmacist before adding yohimbe rather than deciding on your own.

Yohimbe is a West African tree bark that contains yohimbine, a compound mainly promoted for erectile dysfunction and as an aphrodisiac. A prescription form of yohimbine has some evidence for erectile dysfunction, but over-the-counter yohimbe supplements vary widely in content and can cause serious side effects like high blood pressure and fast heart rate. Because of these safety concerns, talk to your pharmacist or doctor before using it.

About Lonafarnib

What Is Lonafarnib Used For?

Brand name: Zokinvy

Zokinvy (lonafarnib) is approved for patients 12 months of age and older who have a body surface area of 0.39 m² or above. It is used to reduce the risk of mortality in Hutchinson-Gilford Progeria Syndrome (HGPS) — a rare, severe premature-aging disorder.

Zokinvy is also used to treat processing-deficient Progeroid Laminopathies — a related group of rare genetic conditions — specifically those caused by a heterozygous LMNA mutation with progerin-like protein buildup, or by homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other Progeroid Syndromes or processing-proficient Progeroid Laminopathies.
