Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the basic idea: your body uses an enzyme called CYP3A4 to help break down many medicines, and Phellodendron contains berberine, which may slow that enzyme down a bit. If it does, a drug cleared by CYP3A4 could build up slightly and have a stronger effect. With Rifampin, the theory points toward modestly higher levels, which might mean more chance of side effects like stomach upset, nausea, or liver irritation.

I'll be honest with you though, this concern is theoretical and comes mostly from lab and early human studies, not strong real-world data. Rifampin is also a powerful enzyme inducer itself, so the picture can get complicated. Since you're treating something serious like TB, please loop in your pharmacist or doctor before adding Phellodendron rather than changing anything on your own.

Phellodendron is a traditional Chinese medicine bark (Huang Bai) that contains berberine and other plant compounds with antimicrobial and anti-inflammatory activity in lab studies. Human evidence for most uses is limited, and it is often sold in combination with other herbs, making its individual effects hard to judge. About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
