Imipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with imipramine and pomegranate. Your body relies on a liver enzyme called CYP2D6 to break imipramine down and clear it out. In lab (test tube) studies, pomegranate appears to slow that enzyme. If that happened in real life, imipramine could linger longer and build up, which might mean more of its usual side effects like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.
The big caveat: this is only seen in lab studies, not in actual people, so we honestly don't know if it matters when you eat the fruit or drink the juice. Food amounts are unlikely to cause trouble, but check with your pharmacist or doctor before using concentrated pomegranate extracts.

Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the short version on pairing pomegranate with imipramine. The theory is that compounds in pomegranate could slow one of the liver enzymes (CYP2C9) that helps break this antidepressant down. If that happened, more imipramine could linger in your body, which in theory might mean stronger side effects like dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat.
The reassuring part: this is mostly a theoretical concern. Lab and animal studies hinted at it, but actual human studies found that pomegranate juice and extract did not meaningfully change this enzyme's activity. So enjoying pomegranate as a food or juice is very unlikely to be a problem. Still, if you're considering concentrated pomegranate extracts, run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Imipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist with imipramine and pomegranate. Your body partly uses an enzyme called CYP3A4 to clear imipramine. In a test tube, pomegranate compounds can slow that enzyme, which in theory could let imipramine build up a bit. If that happened, you might notice more of its usual side effects, like dry mouth, drowsiness, constipation, blurred vision, or a faster heartbeat.
The reassuring part is that real human studies don't back this up. When people drank pomegranate juice, it didn't meaningfully change levels of other CYP3A4 drugs. So enjoying pomegranate as food or juice is very unlikely to cause a problem here. Concentrated extracts are less studied, so if you take one, just give your pharmacist or doctor a heads-up.

What Is Imipramine Used For?
Brand names: Tofranil, Tofranil PM
Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.
Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
