Here's the simple version. BiDil combines isosorbide dinitrate and hydralazine, and both of those work partly by relaxing and widening your blood vessels to lower the workload on your heart. Pomegranate, especially as juice or a concentrated extract, can nudge your blood pressure down a little on its own. Stack the two together and those effects may add up.

What that could feel like is more lightheadedness, dizziness, a headache, or feeling faint when you stand up quickly. This is a theoretical, modest concern based on limited research, not a hard rule, and an occasional glass of juice is very different from taking pomegranate supplement pills daily. Don't change anything on your own. Just give your pharmacist or doctor a heads-up so they can keep an eye on your readings.
