Why this applies to Bosentan

Bosentan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C9.
In vitro research shows that propolis extract can inhibit CYP2C9. However, animal research shows that propolis extract does not significantly affect CYP2C9 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C9 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for Bosentan (Tracleer): your body partly relies on a liver enzyme called CYP2C9 to clear it. In test-tube studies, propolis appears to slow that enzyme down. If that happened in real life, bosentan could build up a bit, which might mean more of its usual side effects, things like headache, flushing, swelling in the legs or ankles, or a drop in blood pressure that leaves you feeling lightheaded.

But I want to keep this in perspective. This is only a lab finding, and animal studies didn't show a real effect, likely because propolis is poorly absorbed. It has never been reported in people. Food or occasional propolis is unlikely to matter, but since bosentan is a serious medication, just run any concentrated supplement by your pharmacist or doctor first.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. Here's the gist with propolis and Tracleer (bosentan). Your liver uses an enzyme called CYP3A4 to clear bosentan from your body. In some lab dish studies, concentrated propolis extract seemed to slow that enzyme down. If that happened in real life, more bosentan could linger in your blood, which in theory might mean more side effects like swelling in your legs, headache, flushing, low blood pressure, or fatigue.

The reassuring part: this is only a theoretical concern. Animal studies and other lab work showed no real effect, and it has never been reported in people. Food-level exposure is very unlikely to matter, but concentrated supplement doses are less studied. Since bosentan is a specialty medicine, just let your pharmacist or doctor know before adding propolis.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
