Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Propolis interacts with.
In vitro research shows that propolis extract can inhibit CYP2C19. However, animal research shows that propolis extract does not significantly affect CYP2C19 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C19 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for Fycompa (perampanel). Propolis, in lab dish studies, can slow down a liver enzyme called CYP2C19, which is one of the pathways that helps clear perampanel from your body. In theory, if that enzyme were blocked, perampanel could build up a bit and lead to stronger side effects like dizziness, sleepiness, trouble with balance, or mood and behavior changes. Perampanel is the active drug itself (not a prodrug), so more buildup means more effect, not less.
The honest part: this concern comes only from test-tube research. Animal studies didn't show a real-world effect, and it has never been reported in people. Everyday food amounts are unlikely to matter, but if you take concentrated propolis, mention it to your pharmacist or doctor before starting.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.

What Is Perampanel Used For?
Brand name: Fycompa
Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.
Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
