Metformin Hydrochloride falls under ANTIDIABETES DRUGS — the group Quassia interacts with.

In animals, quassia extract reduced levels of fasting glucose. Theoretically, quassia might have additive effects when used with antidiabetes drugs. This might increase the risk of hypoglycemia in some patients. Monitor blood glucose levels closely.
Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the gist: in animal studies, quassia seemed to lower fasting blood sugar on its own. Metformin already works to bring your sugar down, so the worry is that the two together could nudge your glucose a little lower than expected. This isn't about quassia changing how your body processes metformin (metformin isn't a prodrug and is cleared by the kidneys, not really broken down). It's simply a possible added blood-sugar-lowering effect.

I want to be honest about the evidence, though. This is based on lab and animal work only, not solid human data, so it's more of a theoretical caution than a proven problem. If you do use both, just keep an eye out for low-sugar signs like shakiness, sweating, or feeling lightheaded, and check your glucose more often. Please loop in your pharmacist or doctor before adding quassia.
