Quercetin and Cefoxitin

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Quercetin interacts with whole categories of medication. Cefoxitin falls into 2 of those categories, so it can interact with Quercetin in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES

1 of 2

Why this applies to Cefoxitin

Cefoxitin falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OAT3 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your kidneys use a little transporter called OAT3 to help flush cefoxitin out of your body. In test tube studies, quercetin can block that transporter. If it slowed clearance in real life, more cefoxitin could linger in your bloodstream than expected, which in theory could nudge up side effects like stomach upset, diarrhea, or, rarely, kidney strain.

The honest part: this is only laboratory and animal data, and it has never actually been reported in people. Food amounts of quercetin are very unlikely to matter. Since cefoxitin is given in the hospital or clinic by injection, your team is already watching you. If you take high-dose quercetin supplements, just mention it to your pharmacist or doctor so they can keep an eye on things.

ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES

2 of 2

Cefoxitin also falls under ORGANIC ANION TRANSPORTER 1 (OAT1) SUBSTRATES.

Theoretically, concomitant use might increase the effects and adverse effects of OAT1 substrates.

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT1, with half-maximal inhibitory concentration (IC50) values less than 10 mcM. So far, this interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Cefoxitin (Mefoxin): your kidneys use a little transporter called OAT1 to pump this antibiotic out of your body into your urine. In lab and animal studies, quercetin appears to block OAT1. If that same effect happened in people, cefoxitin could clear more slowly and build up a bit, which in theory means a little more drug hanging around and a slightly higher chance of side effects like stomach upset, diarrhea, or rash.

I want to be honest about the strength of this: it's theoretical and hasn't actually been reported in humans, and the food amounts of quercetin you'd get from fruits and vegetables are very unlikely to matter. Concentrated high-dose supplements are the bigger question mark. Since cefoxitin is given in the hospital or clinic, just mention any quercetin supplements to your pharmacist or care team so they can keep an eye on things.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Cefoxitin page

“Cefoxitin (Mefoxin) is an injectable antibiotic, usually given in a hospital or clinic for serious infections, and its supplement list deserves genuine care rather than a quick skim. The two entries to understand first are Siberian cocklebur and asarabacca, herbs that can damage the kidneys, adding to a strain cefoxitin can itself plac…”

About Quercetin

What is Quercetin?

Quercetin
  • Seasonal allergies
  • Antioxidant support
  • Heart and blood pressure health
  • Anti-inflammatory support
  • Immune support
  • Exercise recovery

Quercetin is a plant flavonoid with antioxidant and anti-inflammatory properties found in many common foods and sold as a supplement. While early research is interesting for allergies, blood pressure, and inflammation, strong human evidence is limited for most uses. It is generally well tolerated in food amounts, but talk to your pharmacist or doctor before taking high-dose supplements.

About Cefoxitin Injection

What Is Cefoxitin Injection Used For?

Brand name: Mefoxin

Cefoxitin for Injection and Dextrose Injection is used to treat bacterial infections in many parts of the body, including the lungs (pneumonia), urinary tract, abdomen, female reproductive system, bloodstream (septicemia), bones and joints, and skin. It works only against bacteria — not viruses.

Cefoxitin for Injection and Dextrose Injection is also used to prevent infection in people having certain surgeries, including gastrointestinal surgery, vaginal hysterectomy, abdominal hysterectomy, or cesarean section. It does not have activity against Chlamydia trachomatis, so additional medication may be needed when that germ is suspected.
