Why this applies to Methadone

Methadone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the basic idea: methadone is partly broken down by a liver enzyme called CYP2D6, and lab studies suggest quercetin can slow that enzyme down. In theory, if quercetin blocks CYP2D6, your body might clear methadone a little more slowly, letting it build up. With an opioid like methadone, that could mean more drowsiness, dizziness, slowed breathing, or constipation.

The big caveat: this is only seen in test-tube research, not in real people, so I don't want to alarm you. Food amounts of quercetin are very unlikely to matter. But methadone is a powerful drug with a narrow safety margin, so before starting a high-dose quercetin supplement, please run it by your pharmacist or doctor first.

Methadone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the deal with quercetin and methadone. Your liver uses an enzyme called CYP3A4 to help clear methadone from your body. Quercetin can nudge that enzyme, and the research is honestly mixed. Most lab work suggests it slows the enzyme, which could let methadone build up a bit. But one human study showed the opposite, hinting it might speed clearance instead. So we can't say for sure which way it goes.

Methadone is an active drug (not a prodrug), so if levels rise, you might see more drowsiness, slowed breathing, or constipation. If levels drop, you could feel less pain relief or early withdrawal. The evidence here is weak and mostly theoretical, so don't panic, but please check with your pharmacist or doctor before adding quercetin supplements.

About Methadone

What Is Methadone Used For?

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
