Theoretically, concomitant use might increase the levels and adverse effects of CYP2D6 substrates.

In vitro research show that quercetin inhibits CYP2D6. This effect has not been reported in humans.

Here's the idea behind this pairing. In lab dish studies, quercetin can slow down a liver enzyme called CYP2D6. If your body cleared tipranavir mainly through that enzyme, blocking it could let the drug build up and bring on more side effects, like nausea, diarrhea, stomach upset, rash, or changes in your liver labs.

That said, I want to be honest with you: this is only a theory at this point. It comes from test tube research, not from people, and tipranavir is actually handled more by a different enzyme system (CYP3A) along with ritonavir. So a real-world problem here seems unlikely, especially from quercetin in food. Because HIV therapy is too important to guess with, please run any high-dose quercetin supplement past your pharmacist or doctor first.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C9 substrates.

A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.

Here's the basic idea with tipranavir (Aptivus) and quercetin. Quercetin can gently block a liver enzyme called CYP2C9. Tipranavir is partly handled by that same enzyme, so in theory quercetin could slow how fast your body clears the drug, letting levels drift a bit higher. Tipranavir is an active drug, not a prodrug, so higher levels would mean a stronger effect and potentially more side effects, things like nausea, diarrhea, tiredness, or stress on the liver.

I want to be honest about the strength of this, though. It's mostly theoretical, supported by studies on other drugs, not tipranavir itself. Food amounts of quercetin are very unlikely to matter. If you're considering high-dose supplements, check with your pharmacist or doctor first.

P-GLYCOPROTEIN SUBSTRATES

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the quick picture: tipranavir is moved around in your gut by a transporter called P-glycoprotein, which acts like a bouncer pushing some of the drug back out before it gets absorbed. Quercetin may gently block that bouncer. In theory, that could let a little more tipranavir into your blood, which might mean stronger effects and a higher chance of side effects like nausea, diarrhea, stomach upset, or changes in your liver tests.

I want to be honest though: the evidence here is early and mixed. One similar protease inhibitor, saquinavir, wasn't really affected by quercetin at all, so this may not amount to much. Tipranavir is an important part of your HIV treatment, so please don't change anything on your own. Just check with your pharmacist or doctor before starting high-dose quercetin supplements. Food amounts of quercetin are unlikely to be a problem.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your HIV medicine tipranavir is processed by a liver enzyme called CYP3A4, and quercetin can tinker with that same enzyme. The tricky part is that the lab and human studies pull in opposite directions. In some research quercetin seems to block the enzyme (which could let tipranavir build up and raise the chance of side effects like nausea, diarrhea, or liver irritation), while one human study suggests it can actually speed up the enzyme, which could lower drug levels.

Honestly, this is mostly theoretical and based on weak evidence, and food amounts of quercetin are unlikely to matter. But because keeping HIV controlled is so important, please don't add a high-dose quercetin supplement without checking with your pharmacist or doctor first.

What Is Tipranavir Used For?

Brand name: Aptivus

Aptivus (tipranavir) is used to treat HIV-1 infection in adults and in pediatric patients weighing at least 36 kg (about 79 lbs). It is specifically for people who have already tried other HIV treatments and whose virus has become resistant to more than one protease inhibitor. It is always taken together with a low dose of ritonavir and with other HIV medications.

Aptivus is not recommended for people who have never taken HIV treatment before (treatment-naïve patients). Using tipranavir alongside additional active HIV drugs improves the chances of it working well.
