Warfarin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: warfarin is partly cleared by a liver enzyme called CYP2C9, and quercetin can slow that enzyme down. When the enzyme works slower, warfarin can hang around longer in your body. The concern is that your blood could become too thin, which means a higher chance of bruising, nosebleeds, bleeding gums, or more serious bleeding.
Keep in mind warfarin is a narrow-therapeutic-index drug, so even small shifts can nudge your INR. Most of this evidence is theoretical or from studies on other CYP2C9 drugs, not warfarin directly, and food amounts of quercetin are unlikely to matter. Still, before starting a high-dose quercetin supplement, check with your pharmacist or doctor so they can watch your INR.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the practical picture with warfarin. Quercetin can nudge the liver enzymes that help clear certain medications, and lab studies suggest it may slow one of them (CYP3A4). Warfarin is a blood thinner with a very narrow safety window, so even small shifts in how fast your body processes it can matter. If quercetin slows warfarin's breakdown, more could linger in your system, which may thin your blood too much and raise your risk of bruising, nosebleeds, or more serious bleeding.
That said, this concern is mostly theoretical and based on lab and animal data, not strong human studies, and food amounts of quercetin are unlikely to cause trouble. Still, because warfarin is so sensitive, please check with your pharmacist or doctor before starting a high-dose supplement, and keep up with your INR checks.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the practical picture with warfarin and quercetin. The interaction on file is based on lab studies showing quercetin can slow down a liver enzyme called CYP2C8. In theory, blocking that enzyme could let a drug it clears build up a bit. For warfarin, that could mean slightly stronger blood-thinning, which raises the chance of bruising, nosebleeds, or harder-to-stop bleeding.
That said, this is mostly theoretical and rated on weak, lab-only evidence, and warfarin actually relies more on a different enzyme (CYP2C9) than CYP2C8. Still, warfarin is a drug where even small shifts matter, so I wouldn't add a high-dose quercetin supplement without a heads-up to your doctor or pharmacist. Food amounts are very unlikely to cause trouble. If you do start it, ask about checking your INR.

WARFARIN (Coumadin)
4 of 4Animal and in vitro studies show that quercetin might increase serum levels of warfarin. Quercetin and warfarin have the same human serum albumin (HSA) binding site, and in vitro research shows that quercetin has stronger affinity for the HSA binding site and can theoretically displace warfarin, causing higher serum levels of warfarin. Animal research shows that taking quercetin for 2 weeks before initiating warfarin increases the maximum serum level of warfarin by 30%, the half-life by 10%, and the overall exposure by 63% when compared with control. Concomitant administration of quercetin and warfarin, without quercetin pre-treatment, also increased these measures, but to a lesser degree. Researchers theorize that inhibition of CYP3A4 by quercetin may explain these effects. So far, this interaction has not been reported in humans.

Here's the gist with quercetin and warfarin. Warfarin is a tricky blood thinner where even small shifts in its level can matter, so it's one we always watch closely. The concern here is that quercetin might bump up the amount of active warfarin floating around in your blood, partly by nudging it off the proteins it normally rides on and possibly by slowing one of the liver enzymes that clears it. If that happens, warfarin's effect could get stronger, meaning a higher INR and more bleeding risk (easy bruising, nosebleeds, bleeding gums, or blood in urine or stool).
That said, this is mostly theoretical, based on lab dishes and animal studies, and it hasn't actually been reported in people. Quercetin you get from foods like apples and onions is very unlikely to be a problem. Concentrated supplements are the real question, so check with your pharmacist or doctor before starting one, and keep up with your regular INR checks.

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
