Why this applies to Dronedarone

Dronedarone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Quercetin interacts with.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the gist: your body uses an enzyme called CYP3A4 to break down dronedarone (Multaq), and quercetin can nudge that enzyme. In most lab and small human studies, quercetin seems to slow CYP3A4, which could let more dronedarone build up in your blood. Dronedarone isn't a prodrug, so higher levels mean stronger effects and a bigger chance of side effects like a slow heartbeat, dizziness, tiredness, stomach upset, or QT changes on an EKG.

The honest part: this is mostly theoretical and based on lab and animal work, with mixed human results. Food amounts of quercetin are very unlikely to matter, but concentrated supplements are a different story. Since dronedarone is a sensitive heart drug, please check with your pharmacist or doctor before adding a high-dose supplement.

Dronedarone also falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the idea behind this pairing. Your body uses a liver enzyme called CYP2C8 to help clear dronedarone, and in test-tube studies quercetin can slow that enzyme down. If that happened in real life, dronedarone could build up a bit more than usual. Since dronedarone is an active drug (not a prodrug that needs switching on), more of it sticking around could mean stronger effects, like slower heart rate, dizziness, tiredness, or stomach upset.

That said, this is largely theoretical. The concern comes from lab work, and one human study with a similar drug showed no real effect. Food amounts of quercetin are very unlikely to matter, but if you're considering high-dose supplements, please check with me or your doctor first, since dronedarone affects the heart.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
