Amitriptyline, Perphenazine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Your liver uses several enzymes to clear both, and CYP3A4 is one of them. Lab studies suggest oridonin, a compound in Rabdosia rubescens, may speed up CYP3A4. In theory, that could lower the levels of these medicines in your body, meaning your depression or other symptoms might not stay as well controlled.

I want to be honest with you: this is based only on test-tube and animal research, and it has never been reported in people. So I wouldn't lose sleep over it. Still, since this herb hasn't been well studied for safety, please check with your pharmacist or doctor before adding it, and don't change your medicine on your own.

Amitriptyline, Perphenazine also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 2C9 (CYP2C9) enzyme. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the situation with this combination pill (which contains amitriptyline plus perphenazine). In test-tube studies, a compound in Rabdosia rubescens called oridonin seems to speed up an enzyme called CYP2C9, one of the enzymes that helps clear amitriptyline. If that effect happened in real life, your body might break down the amitriptyline a little faster, which could mean slightly less benefit for things like mood or nerve pain.

The good news: this is very early evidence. It has only been seen in lab dishes, never in actual people, and it's rated a minor interaction. I wouldn't lose sleep over it. Still, if you notice your symptoms creeping back, mention it to your pharmacist or doctor before changing anything.
