Imipramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for imipramine (Tofranil). Your body partly uses an enzyme called CYP3A4 to break this antidepressant down. Lab and animal research hints that oridonin, a compound in Rabdosia rubescens, may speed up that enzyme. If that happened in people, your body could clear imipramine a little faster, which might lower its levels and make it work less well. For someone treating depression, that could mean symptoms creeping back; for a child using it for bedwetting, it may mean fewer dry nights.

I'll be honest though, this is only test-tube and animal data, with nothing reported in actual patients, so the real-world risk is uncertain and probably small. Imipramine still needs careful dosing, so loop in your pharmacist or doctor before adding this herb.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 2C9 (CYP2C9) enzyme. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for imipramine (Tofranil). This herb contains a compound called oridonin that, in test-tube studies, seems to speed up a liver enzyme called CYP2C9. Imipramine is partly cleared by that enzyme (along with several others). So in theory, taking Rabdosia rubescens could help your body break imipramine down a bit faster, lowering its level. That might make it work less well, so your depression could feel less controlled, or in a child being treated for bedwetting, the benefit could fade.

The reassuring part: this is only lab evidence and hasn't been seen in real people, so it's a minor, possible concern, not a proven one. Still, since imipramine needs steady levels to work, it's worth a quick chat with your pharmacist or doctor before adding this supplement.

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
