Here's the gist with Mersyndol (acetaminophen, codeine, and doxylamine) and rauwolscine. The codeine and doxylamine pieces can both nudge the brain's seizure threshold a little lower, especially at higher doses, and rauwolscine is a stimulant that may do the same. Stacking them together could, in theory, make a seizure slightly more likely in someone who is already prone to them.

I want to be honest about how thin this evidence is: it comes from one animal study, not from people, so for most folks the real-world risk is probably low. Still, since rauwolscine can also rev up heart rate, blood pressure, and anxiety, it's worth a quick chat with your pharmacist or doctor before combining the two, particularly if you have any seizure history.

Rauwolscine is structurally related to yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the wrinkle with this combo. The codeine in Mersyndol is a prodrug, meaning your body has to switch it on using an enzyme called CYP2D6 before it can relieve pain. Rauwolscine is related to yohimbine, and lab studies suggest yohimbine can block CYP2D6. If that happens here, less of your codeine gets converted to its active form, so you may get weaker pain relief than expected.

I want to be honest about the strength of this: it's theoretical, based on test-tube data, not real people. So I wouldn't panic. Still, if you notice your pain isn't being controlled, or you feel jittery or anxious from the stimulant, please check in with your pharmacist or doctor before changing anything.

Rauwolscine (alpha-yohimbine) is a stimulant alkaloid closely related to yohimbine that is marketed mainly in fat-burning and pre-workout supplements. Human evidence for its benefits is very limited, and it can raise heart rate and blood pressure and cause anxiety, so it should be used with caution and is not recommended for people with heart conditions, anxiety, or during pregnancy or breastfeeding.
