Here's the gist with Fiorinal with Codeine, which combines aspirin, butalbital, caffeine, and codeine. The concern isn't really about how these get broken down. It's that rauwolscine, a stimulant cousin of yohimbine, may lower the seizure threshold, meaning it could make seizures a little easier to trigger. Stacking it with the caffeine in this product (also a stimulant) leans the same direction.
To be honest, this is a theoretical worry based only on animal lab work, where rauwolscine lowered the seizure threshold for a test chemical. For most people taking this medication as directed, the practical risk is low. Still, if you have a seizure history, or notice racing heart, jitteriness, or anxiety, please loop in your pharmacist or doctor before combining them.

Rauwolscine is structurally related to yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the wrinkle with this combo product: one of its ingredients, codeine, is a prodrug. It doesn't do much on its own. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Rauwolscine is a cousin of yohimbine, and lab studies suggest yohimbine can slow down that same CYP2D6 enzyme.
So in theory, rauwolscine might block codeine from converting to morphine, meaning you could get less pain relief than expected. Keep in mind this is only a theoretical concern based on lab data, not real-world reports. There's also a separate issue worth mentioning: rauwolscine and the caffeine here are both stimulants, so together they may bump up heart rate, blood pressure, and jitteriness. If you take Fiorinal with Codeine regularly, run this by your pharmacist before adding the supplement.

Here's the piece worth knowing: your medication contains aspirin, which already thins the blood and makes platelets less sticky. Rauwolscine may, in theory, do a little of the same thing. Lab studies on its cousin yohimbine show it can block platelets from clumping together. Stack that on top of aspirin and you could, at least on paper, raise your bleeding risk a bit. That might look like easier bruising, nosebleeds that take a while to stop, or bleeding gums.
I want to be honest about the strength of this: it's theoretical, drawn from test-tube research, not from people. It's not a reason to panic. Still, since rauwolscine also pushes up heart rate and blood pressure, I'd run it by your pharmacist or doctor before combining the two.
