Why this applies to Isavuconazonium Sulfate (oral)
Isavuconazonium Sulfate (oral) falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for anyone taking Cresemba (isavuconazonium) alongside rhodiola. Your body clears this antifungal partly through a liver enzyme called CYP3A4. In lab (test tube) studies, rhodiola can block that enzyme a little, and in theory that could nudge isavuconazole levels up. Higher levels might mean more of its usual side effects, like nausea, headache, or a shift in liver blood tests.
That said, the real-world evidence here is weak. The effect varies a lot by product, and a human study using a similar enzyme test drug found no meaningful change. So this is mostly theoretical and rated minor. Because you're treating a serious fungal infection where steady drug levels matter, don't add rhodiola without first checking with your pharmacist or doctor.
