Rhodiola and Rituximab

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Rituximab

Rituximab falls under IMMUNOSUPPRESSANTS — the group Rhodiola interacts with.

Theoretically, rhodiola use might interfere with immunosuppressive therapy.
In vitro and animal research show that rhodiola has immunostimulatory effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this one. Rituximab (Rituxan) works by calming down or knocking out certain immune cells, which is exactly what you want when treating lymphoma, leukemia, or rheumatoid arthritis. Rhodiola, on the other hand, has shown immune-stimulating effects in lab and animal studies. So in theory, an herb that revs the immune system up could work against a medicine that is meant to dial it down, possibly making your treatment less effective.

I want to be honest about the strength of this, though. It's only based on test-tube and animal data, not real patients, so it's more of a caution than a proven problem. Rhodiola isn't broken down by Rituximab or vice versa, so this isn't a metabolism issue. Still, with a serious therapy like this, it's worth a quick chat with your oncologist or pharmacist before adding Rhodiola.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

From our pharmacist’s overview on the Rituximab page

“Rituximab (Rituxan) works by knocking down part of the immune system, clearing out the B cells that drive lymphomas, leukemias, and autoimmune diseases like rheumatoid arthritis. All 53 supplements here interact the same way, showing some evidence of stimulating immune activity that could in theory blunt what the drug is trying to do.”

About Rituximab Injection

What Is Rituximab Injection Used For?

Brand name: Rituxan

Rituximab is used to treat a range of blood cancers and immune-driven diseases. In cancer care, it targets certain types of Non-Hodgkin's Lymphoma (NHL) and Chronic Lymphocytic Leukemia (CLL) — both in adults and, for some lymphomas, in children as young as 6 months old.

Beyond cancer, rituximab is also approved for moderately-to-severely active Rheumatoid Arthritis (RA) in adults who haven't responded well to other treatments, as well as for two rare inflammatory blood vessel diseases — Granulomatosis with Polyangiitis (GPA) and Microscopic Polyangiitis (MPA) — and for a serious skin blistering condition called Pemphigus Vulgaris (PV).
