Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.

Treprostinil (Orenitram) is a medicine for pulmonary arterial hypertension, and one of its main jobs is to widen blood vessels. That action already tends to lower your blood pressure and can cause things like dizziness, lightheadedness, flushing, headache, or feeling faint when you stand up. The thought here is that Rhodiola might add a little extra blood-pressure-lowering on top of that, so those effects could feel stronger.

I want to be honest about the evidence though: this is only a theoretical concern based on lab and animal studies, not real patient data, so it may not amount to much in practice. Still, since Treprostinil's dose is carefully adjusted, it's worth a quick chat with your pharmacist or doctor before adding Rhodiola, and let them know if you notice more dizziness.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
