Vincristine Sulfate falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist for vincristine. Your body uses a kind of cellular bouncer called P-glycoprotein to pump this chemo drug back out and keep its levels in check. In lab dish studies, rhodiola appears to slow that pump down. If the pump works less well, more vincristine could linger in your system than expected.

With vincristine, higher levels can mean more of its known side effects, things like numbness or tingling in the hands and feet, constipation, muscle weakness, or nerve pain. I want to be honest though: this concern comes only from test-tube research, not from people, so it's theoretical. Still, chemo is serious business, so please don't add rhodiola without first clearing it with your oncologist or pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Vincristine Sulfate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the basic idea: your body clears vincristine using a liver enzyme called CYP3A4. In lab dishes, rhodiola can slow that enzyme down a little, which could in theory let vincristine build up. Since vincristine isn't a prodrug, more of it hanging around means a stronger effect and a higher chance of its known side effects, things like nerve tingling or numbness in the hands and feet, constipation, jaw pain, or low blood counts.

That said, the real-world evidence here is thin. A human study using a similar CYP3A4 test drug found no meaningful change. Still, vincristine is potent chemotherapy, so don't add rhodiola without first checking with your oncologist or pharmacist.
