Why this applies to Modafinil

Modafinil falls under P-GLYCOPROTEIN SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for sage and modafinil. Sage may block a tiny pump in your gut and elsewhere called P-glycoprotein, which helps shuttle certain drugs out of your body. If that pump slows down, a bit more modafinil could stay in your system. In theory that might nudge up modafinil's usual effects, things like trouble sleeping, headache, jitteriness, a racing heart, or an upset stomach.

I want to be honest with you, though: this is only seen in lab dish testing, not in real people. It hasn't been shown to actually cause problems. Cooking with sage is almost certainly a non-issue. If you take concentrated sage capsules or the oil, just mention it to your pharmacist or doctor so they can keep an eye out.

Modafinil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist: your body partly uses an enzyme called CYP3A4 to break down modafinil and clear it out. Lab tests suggest sage may slow that enzyme down a bit. If that happened in people, modafinil could hang around a little longer and feel stronger, which might mean more of its usual effects like jitteriness, headache, trouble sleeping, a faster heartbeat, or feeling on edge.

I want to keep this in perspective though. This is only a theoretical concern from test-tube studies, and it has never actually been reported in real patients. A little sage in your cooking is very unlikely to matter. Concentrated sage extracts or the essential oil are the bigger question. If you take those regularly, just mention it to your pharmacist or doctor so they can keep an eye on how you feel.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
