Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with SAM-e and your levodopa/carbidopa. SAM-e works as a methyl donor, meaning it hands off small chemical groups to other molecules. The worry is that it may add one of these groups onto levodopa itself, which could make the levodopa less able to do its job. Since levodopa is what your body converts into dopamine to control Parkinson's symptoms, weaker levodopa could mean more stiffness, tremor, slowness, or trouble moving.

I want to be honest about the evidence here: this is mostly based on lab and animal work, not solid human studies, so it's more of a theoretical caution than a proven problem. Still, please don't start SAM-e without first checking with your doctor or pharmacist so we can keep your Parkinson's well controlled.

This page covers Same only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Levodopa, Carbidopa page lists 26 supplements it has a documented interaction with, worst first, each one explained.

By severity: 7 major · 17 moderate · 2 minor.

From our pharmacist’s overview on the Levodopa, Carbidopa page

“Cowhage is the first thing to know here. It naturally contains levodopa, sometimes 75 to 400 mg per dose, so stacking it on Levodopa, Carbidopa (Duodopa) risks levodopa excess.”
