Here's the thing with treprostinil (Orenitram): besides relaxing blood vessels for pulmonary hypertension, it can mildly affect your platelets and slow clotting on its own. Saw palmetto has been reported to lengthen the time it takes your blood to clot too. So when you pair the two, the worry is simply additive: you could bruise a little easier or bleed a bit longer than expected.

I want to keep this in perspective, though. This concern rests on anecdotal reports and theory, not strong studies, so it's a possibility rather than a sure thing. Still, watch for signs like nosebleeds, bleeding gums, or unusual bruising. Before starting or stopping saw palmetto, please run it by your doctor or pharmacist first.

The best research suggests it works no better than a placebo for most men, though it is generally well tolerated. About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
