Amitriptyline, Perphenazine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic). Your body partly uses an enzyme called CYP2C19 to break down amitriptyline. Schisandra appears to slow that enzyme down, at least in lab and animal studies. In theory, that could let amitriptyline build up a bit higher than usual, which may mean stronger effects like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.

Here is the honest part: this has not been shown in real people, only in test tubes and animals, so it is more of a caution than a proven problem. Still, since these are not gentle medicines, I would run it by your pharmacist or doctor before adding schisandra.

This combination pill pairs amitriptyline (an antidepressant) with perphenazine (an antipsychotic). The concern on file is that schisandra may rev up a liver enzyme called CYP2C9, which helps clear amitriptyline. In theory, speeding up that enzyme could lower the drug level, meaning your medication might not work quite as well. For something used to steady mood, that could show up as a return of depression symptoms or your usual control slipping.

Here's the honest part: this is based only on lab and animal studies, and it has never been seen in real people. So I wouldn't lose sleep over it. Still, since this medicine matters, mention any schisandra supplement to your pharmacist or doctor before adding it.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

This combination pill (Triavil) pairs amitriptyline, an older antidepressant, with perphenazine, an antipsychotic. Both rely partly on a liver enzyme called CYP3A4 to be cleared from your body. Schisandra appears to slow that enzyme down, so the medication can build up to higher levels than expected.

In real terms, that could mean stronger or more noticeable side effects, things like heavy drowsiness, dry mouth, constipation, dizziness on standing, a faster heartbeat, or trouble urinating. Keep in mind this concern comes mostly from lab and animal data, not solid human studies with this exact drug, so the real-world risk isn't certain.
