Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the wrinkle with this combo: the codeine part of your pill is actually a prodrug. Your body has to switch it on using a liver enzyme called CYP2D6 before it relieves pain. Lab and animal studies hint that a compound in Schizonepeta may slow that same enzyme down. If that happened in people, less codeine would get converted to its active form, so you might get weaker pain relief than expected.
I want to be honest with you, though: this is only theoretical, based on test-tube and animal research, not real patients. The acetaminophen and butalbital pieces aren't the concern here. Before you pair them up, just run it by your pharmacist or doctor so they can check the rest of your medicine list.

Here's the honest version for this combo pill (acetaminophen, butalbital, and codeine). The concern on file is that a compound in Schizonepeta may slow down a liver enzyme called CYP1A2. But this is based only on lab and animal research, so it's theoretical, not something we've seen play out in people.
Practically speaking, CYP1A2 isn't the main pathway for any of these three ingredients. Codeine relies mostly on CYP2D6, butalbital and acetaminophen go through other routes, so the real-world overlap here is small. Still, if this combination were nudged up at all, you might feel more drowsy, dizzy, lightheaded, or constipated. Since acetaminophen is hard on the liver in large amounts, never exceed your dose. Please run the Schizonepeta past your pharmacist or doctor before mixing them.

Some substrates of CYP2E1 include acetaminophen, chlorzoxazone (Parafon Forte), ethanol, theophylline, and anesthetics such as enflurane (Ethrane), halothane (Fluothane), isoflurane (Forane), and methoxyflurane (Penthrane).

Here's the piece that matters for this combo pill: the acetaminophen part is partly broken down by a liver enzyme called CYP2E1. Lab and animal work hints that a compound in Schizonepeta may slow that enzyme down. In theory, that could let acetaminophen hang around a bit longer. The bigger concern isn't extra pain relief, it's the liver, since one of acetaminophen's breakdown steps through CYP2E1 affects how the liver handles it.
Honestly, this is theoretical and based only on animal data, so I wouldn't lose sleep over it. The butalbital and codeine parts aren't the focus here. The real-world rule still holds: don't go over your daily acetaminophen limit, skip alcohol, and check with your pharmacist or doctor before adding Schizonepeta.
