Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for Alpelisib (Piqray). Your body clears this cancer drug partly through a liver enzyme called CYP3A4. A compound in Schizonepeta, called schizonepetin, appears to speed up that enzyme, at least in animal and lab studies. If that holds true in people, your body could break down Alpelisib faster, leaving less of it in your blood. Since Alpelisib is the active drug (not a prodrug that needs switching on), faster clearance could mean a weaker anticancer effect, which is the last thing you want during treatment.

I'll be honest: this is theoretical and only based on early research, not human studies. Still, with a drug this important, I wouldn't gamble. Please run any Schizonepeta-containing product past your oncologist or pharmacist first.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
