Dextromethorphan, Pseudoephedrine, Triprolidine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Of the three ingredients in Actifed DM, the one to watch here is the dextromethorphan (the cough suppressant). Your liver uses an enzyme called CYP2D6 to clear it. Lab and animal studies hint that a compound in Schizonepeta may slow that enzyme down. If that happens in people, dextromethorphan could build up a bit more than usual, which might mean more drowsiness, dizziness, or in rarer cases a jittery, overstimulated feeling.

I want to be honest about how thin the evidence is. This comes only from test-tube and animal research, not real patients, so it may not amount to much in everyday life. Still, if you're already feeling extra sleepy or off, that's worth mentioning. Before combining them, just run it by your pharmacist or doctor so they can look at your full medication list.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Actifed DM combines three actives, and one of them, dextromethorphan (the cough suppressant), is partly broken down by a liver enzyme called CYP3A4. Lab and animal research hints that a compound in Schizonepeta may speed that enzyme up. In theory, that could clear dextromethorphan a bit faster and make it a little less effective for calming your cough. The pseudoephedrine (decongestant) and triprolidine (antihistamine) aren't really driven by this same pathway, so they're less of a concern here.

Keep in mind this is only test-tube and animal data, not human evidence, so any real effect is uncertain and likely small. Food-style amounts of the herb are unlikely to matter much. Still, it's worth a quick check with your pharmacist before combining them.
