Schizonepeta + Doravirine
Doravirine brand name: Pifeltro.
Schizonepeta has a moderate interaction with Doravirine. Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Doravirine
Doravirine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schizonepeta interacts with.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea for doravirine (Pifeltro). Your body relies on a liver enzyme called CYP3A4 to handle this HIV medicine. Animal and lab research hints that a compound in Schizonepeta may speed that enzyme up. If that happened in people, doravirine could be cleared from your blood faster, which might lower its levels and weaken how well it controls your HIV.
That matters because doravirine works best at steady levels. If it dips too low, the virus has more room to multiply, and that can even lead to resistance over time. I want to stress this is theoretical, based only on lab and animal data, so we don't truly know if it happens in humans. Still, with HIV therapy I'd play it safe. Please check with your pharmacist or doctor before combining the two.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Doravirine page
“Doravirine (Pifeltro) is an HIV medicine, and keeping its blood level steady is what keeps the virus suppressed, so this list of 138 interacting supplements deserves real vigilance. Three entries are rated major, and St.”

About Schizonepeta
What is Schizonepeta?
- Common cold and early-stage fever
- Itchy skin rashes and hives
- Sore throat
- Headache
- Measles and other skin eruptions (traditional use)
Schizonepeta is a mint-family herb long used in traditional Chinese medicine, usually as part of combination formulas for colds, fevers, and itchy skin conditions. Modern human evidence is very limited, so its benefits are not well proven. Talk to a pharmacist or doctor before using it, especially if you are pregnant, breastfeeding, or taking other medicines.
About Doravirine
What Is Doravirine Used For?
Brand name: Pifeltro
Pifeltro (doravirine) is used together with other antiretroviral medications to treat HIV-1 infection in adults and in pediatric patients weighing at least 35 kg (about 77 lbs). It is approved for people who have never been on HIV treatment before, as well as for people who are already virologically suppressed — meaning their HIV is currently undetectable — and want to switch to a new stable regimen.
Pifeltro is not used alone; it must always be part of a combination HIV regimen. Patients switching to a Pifeltro-based regimen must have no history of treatment failure and no known resistance to doravirine.
