Ibuprofen, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the wrinkle with this combo product. The hydrocodone part actually relies on an enzyme called CYP2D6 to turn into its more potent painkilling form. Schizonepeta is thought, based only on animal and test-tube work, to slow that same enzyme down. So in theory it could mean less conversion to the active form, which might make the hydrocodone feel less effective for pain.

The truth is, this is weak, early evidence, and a true picture isn't fully clear since blocking CYP2D6 can also leave more parent hydrocodone around. The ibuprofen side isn't really involved here. I wouldn't lose sleep over it, but since pain relief and opioid effects matter, run it by your pharmacist or doctor before pairing them.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for your Ibucodone (ibuprofen plus hydrocodone). Schizonepeta contains a compound that, in animal and lab studies only, appears to speed up a liver enzyme called CYP3A4. Hydrocodone is partly handled by that enzyme, so in theory revving it up could clear the hydrocodone faster and leave you with less pain relief than expected. Interestingly, hydrocodone is also a prodrug that becomes its stronger form (hydromorphone) through a different enzyme, CYP2D6, so the overall effect here is hard to predict.

Honestly, this is a weak, theoretical concern based only on animal data, and the ibuprofen part isn't really affected. I wouldn't lose sleep over it, but do mention the herb to your pharmacist or doctor before combining them, especially since this is a narcotic combo.
